The service integrates an empathetic approach to addressing any concerns and frustrations an individual or rehab team may have, due to the persons physical and mental health, providing a proactive and preventative service to best avoid reactive measures and reducing the possibility of seeking primary care services.

A rehabilitation Nurse will:
- Assess and evaluate clients to determine the cause and effects of illnesses, diseases and conditions liaising closely with consultants and GP’s
- Liaise with the MDT to produce individualised care plans
- Educate and support clients and their families to adjust to a different way of life
- Identify any health hazards and empower individuals to thrive
- Focus on recovery optimisation
- Provide counselling to clients and their families in a bid to help both come to terms with the changes and to best manage their lives and decision-making responsibilities.
- Effectively implement plans of care directly to maintain and restore functions and prevent complications in the future as well as educate clients support workers.
